weapon disappearing from inventory

My axes dissapeared from my inventory after i died, went back got my bag and they weren't there. Then my mace vanished while I was in my castle.

I have had this happen twice with Ancestral Maces while traveling in the Cursed Forest

Crafted weapons and shattered weapons that have been reforged disappears when moving in/out of chests and now completely gone. Also gear disappeared after unequipping servants who then were unsummoned.
