Maja peculiarities

I hadn't fought Maja on brutal before this patch, so I'm unsure if this is new. But I had some frustrations that I suspect weren't intended.

1. Repeatedly when I fought her, ran away or died, and then came back to start a new fight, she was doing phase 2 abilities right away: summoning minions, doing the wide diagonal "paper cut" book attacks, even the "come to me" healing, when she was only down to 80% health. Seems like she is not resetting properly.

2. Her leash range seemed strange. One time after I was almost dead so ran away outside to end the fight, she still did the mass minion summoning move where she disappears, and a minion spawner appeared under me in the grass outside, which I couldn't see, and killed me. Not a great experience when you think the fight is ending and also can't see the ground to even know you need to dodge.
