The in-game voice chat disabled sometimes

We were enjoying the game by using the in-game voice chat to communicate, however, there are two main issues occur in some of the players.
1. The in-game voice chat setting sometimes automatically turns to "off", so it has to be changed to close-range voice chat manually when it happens.
2. Some of the players can talk to other people but cannot hear others, only re-enter the server will make them hear.
Does anyone have similar problems and how to fix it? We are all streamers so it is really annoying when these problems occur.
