Ancestral Forge glitches

-What exactly is happening?
Like someone else on here, I also had my Ancestral weapon "disappear" but it disappeared by turning into a Merciless Iron weapon. Not only that, but it produced a whole bunch of Merciless Iron copies of that weapon and no, I don't have any mods or cheats active. I was trying to upgrade my weapon on the Ancestral Forge and it put a bunch of Merciless Iron copies in my inventory but in the forge it produced multiple different spear weapons with different effects instead of just upgrading my weapon. I don't know how it happened.
-What were you doing before the issue occurred?
Literally I just built the Ancestral Forge and it had been working fine the first time I put the shards together, but it only did the item duplication glitch and the multiple different spears when I tried to upgrade my weapon. I used the Devourer on the copies to clear my inventory.
-When did the issue start? Was it after a specific update or event?
I don't think so. The night before had been a Blood Moon but that shouldn't affect anything and likewise an active rift somewhere else shouldn't affect anything.
-Have you modified your game files or installed any mods?
Again, no mods, no cheats, and no console commands.
I've restarted Steam and the game but haven't restarted my PC because I really don't think it's on my end.
